QUESTION: Did your certificate expire last June 30th...or...did you surrender your certificate early?
If your answer was "YES"...then another question: Have you submitted your recertification plan to the LPDC (by sending or delivering it to the Central Office and having it date-stamped by office personnel)?
If "YES"...then the process will take care of everything else.
If "NO"...then you have work to do...SOON! The deadline for submitting your recertification plan is April 30th...only 18 days away.
PLEASE...DON'T MISS THIS DEADLINE!
